Thank you for asking us to get your enhanced DBS certificate arranged for you. The cost is £59.50 and we will endeavour to get this done for you as soon as possible. We don’t have a direct influence on the speed of the check, but we can inform you how far the application you are. We will also chase for you, when we can.

Typically though, a Passport, driving licence, and utility bill, are perfect.

WHAT HAPPENS NEXT:

1. Complete the form below and make the payment
2. There will be a page for you to copy a PIN, and take note of the business name too (PHAT Services Ltd) You will need this information for the DBS Application.
3. Complete the DBS Application
4. We will receive notification of your application and verify what you have sent in below.
5. You will receive login details so you can track your application (failing that…just call or email us)
6. We then wait for your application to be processed
7. If you don’t hear anything after 90 working days, we will chase up your DBS application (please note, we cannot chase for you until this point)
8. As soon as your DBS certificate has gone through, you will be sent your DBS certificate by post, directly.
9. We STRONGLY recommend that as soon as you have your certificate, to sign up for the DBS update service. This will mean that you won’t have to wait lengthy periods and people who need to access your information, can do so, very quickly.

• Passport – Any current and valid passport
• Current driving licence photocard – (full or provisional) – UK, Isle of Man, and Channel Islands. From 8 June 2015, the paper counterpart to the photocard driving licence will not be valid and will no longer be issued by DVLA
• Birth certificate – issued within 12 months of birth – UK, Isle of Man and Channel Islands – including those issued by UK authorities overseas, for example embassies, High Commissions and HM Forces
• Current biometric residence permit – UK
• Adoption certificate – UK and Channel Islands

• Birth certificate – issued after time of birth, UK, Isle of Man and Channel Islands
• Non UK current driving licence photocard – (full or provisional) All countries outside the UK (excluding Isle of Man and Channel Islands)
• Firearms licence – UK, Isle of Man and Channel Islands
• Current driving licence (full or provisional) – paper version – (if issued before 1998) – UK, Isle of Man, and Channel Islands
• Immigration document, visa or work permit – Issued by a country outside the UK. Valid only for roles whereby the applicant is living and working outside of the UK. Visa/permit must relate to the non-UK country in which the role is based
• Marriage/civil partnership certificate – UK and Channel Islands

You must have AT LEAST 1 from here. You can choose a maximum of 3 from this section. If you choose all 3, then you will not need to present any more identification from List 3.

List 3 *

• Bank or building society statement – UK and Channel Islands Issued in last 3 months.
• Cards carrying the PASS accreditation logo – UK, Isle of Man and Channel Islands Must still be valid.
• Letter from head teacher or college principal – UK – for 16 to 19 year old in full time education. Only used in exceptional circumstances if other documents cannot be provided. Must still be valid.
• P45 or P60 statement – UK and Channel Islands Issued in last 12 months.
• Benefit statement – e.g. Child Benefit, Pension UK Issued in last 3 months.
• Mortgage statement – UK Issued in last 12 months.
• Council Tax statement – UK and Channel Islands Issued in last 12 months.
• Non-UK Bank or building society statement – Countries outside the UK. Branch must be in the country where the applicant lives and works. Issued in the last 3 months.
• Financial statement – e.g. pension or endowment UK Issued in last 12 months.
• Utility bill – UK – not mobile telephone bill. Issued in last 3 months.
• Bank or building society account opening confirmation letter – UK Issued in last 3 months.
• Credit card statement – UK Issued in last 3 months.
• EEA National ID card – Must still be valid.
• Letter of sponsorship from future employment provider – Non-UK only – valid only for applicants residing outside of the UK at time of application. Must still be valid.
• Central or local government, government agency, or local council document giving entitlement – e.g. from the Department for Work and Pensions, the Employment Service, HMRC UK and Channel Islands. Issued in last 3 months.
• Irish Passport Card – Cannot be used with an Irish passport Must still be valid.

You can choose up to a maximum of 2 from this section. You do not need more than 3 forms of ID. If you have only got 1 from list 1, you will need to provide 2 from here. If you only have 1 from list 2, you will need to provide 2 from here. If you have selected 2 x forms of ID from either List 1 or 2, then you will only need to provide 1 from this list.
